The Chronic Disease Self-Management Program is a series of workshops offered once a week, for six weeks, for two hours by two trained leaders. These workshops are for people with a chronic condition or those who care for someone with a chronic condition.
Topics that are covered in these workshops include:
- Ways to deal with frustration, fatigue, pain and isolation
- Exercises for maintaining and improving strength, flexibility, and endurance
- Communicating with family, friends, and health care professionals
- Decision making
