The Chronic Disease Self-Management Program is a series of workshops offered once a week, for six weeks, for two hours by two trained leaders. These workshops are for people with a chronic condition or those who care for someone with a chronic condition.
Topics that are covered in these workshops include:
- Ways to deal with frustration, fatigue, pain and isolation
- Exercises for maintaining and improving strength, flexibility, and endurance
- Communicating with family, friends, and health care professionals
- Decision making
Participants are also shown ways to break symptom cycles and make action plans. You can view the schedule for the classes here. If you would like to join or be trained to lead a Chronic Diabetes Self-Management Workshop, contact Sherry Ellem at 304-485-7374 ext. 151.
