Why Report

Disease reporting in West Virginia is required by the West Virginia Title 64 Legislative Rule also known as the Communicable Disease Rule. View the Rule (PDF).


When, and How to Report

Cases of reportable disease in West Virginia residents should be reported on a West Virginia Reportable Disease Card (also know as a Yellow Card). When reporting be sure to complete the entire front of the card, and when available include any clinical symptoms or pertinent history in the comments section.
