Environmental Service Fees
SG-55 for NEW INSTALLATIONS ONLY – $50
The SG-55 Form can be completed when requested. However, the only references that can be made are in relation to the INSTALLATION OF THE SYSTEM. All questions regarding dye testing and functioning of the system are to be marked as “Not Evaluated”;
The Comments Section shall include the following statement:
“No assessment as to the functioning of the system was conducted nor can be inferred from the information provided by this office. The information is solely intended to provide information concerning the installation of the septic tank, aeration unit, and/or absorption field.”
Homeowners Sewage Installation Test – $50
Individuals may install their own Class II septic tank system (septic tank and/or absorption field) upon passing the standardized State exam. Each attempt to pass the exam is considered a separate test requiring a separate fee.
MOVHD Water Sample Fee – $20
This fee is applicable for when requested to take water samples. It does not apply to a water sample requested as part of a newly permitted and installed well, or water samples taken as part of routine surveillance.
Proposed Water Sample Fee: $50.00
MOVHD Chain of Custody Water Sample Fee – $20
This fee is applicable for when requested to take water samples that require a chain of custody be kept. This would apply to samples taken for a Water Well before Oil and Gas well drilling,so that there would be a representative sample before the drilling occurs to show water well changes because of drilling. It does not apply to a water sample requested as part of a newly permitted and installed well, or water samples taken as part of routine surveillance.
Proposed Chain of Custody Water Sample Fee: $75.00
Food Establishment Plan Review Fee – Sliding Scale
Plan Review Fees conducted as part of the permitting process of a food establishment will be based on the seating capacity of the proposed establishment.
0 – 35 Seats $ 50
36 – 75 Seats $100
76 Seats + $150
The fee is required prior to conducting the plan review and is separate from the Fee for Permit.
Food Establishment Re-Inspection Fee – $25
West Virginia requires food establishments to be inspected twice a year (One inspection in each half of the physical year.) These are considered “routine” inspections and are not subject to Re-Inspection Fee.
The criteria for requiring re-inspection of a food establishment are defined in Sections 8-405.11, 8-405.20 and 8-405.11 of the Food Code. As a general summary of these sections:
CRITICALS not corrected during the inspection = Maximum 10 day Notice
NON-CRITICALS not corrected during the inspection = Maximum 90 day Notice
Re-Inspection Fees will be billed according to the information supplied on the Food Establishment Questionnaire, as submitted for permitting purposes each year.
All re-inspection fees will be billed from the Parkersburg office by Tami Davis.
There is a $5 Fee for Service charge to individuals receiving foodhandler training from MOVHD.
The fee must be paid prior to receiving the training and card. There is no billing available for this service.
